How To Enrol

• Print Enrolment Contract (Date, Indicate Company Name and Sign) (click here)
• Print Personal Data Sheet (one for each employee) (click here)
  Can be completed on-line and e-mailed to Shield Medical (especially useful for adding new employees)
• Mail original receipts to Edmonton Head Office along with a company cheque made payable to Shield Medical for initial set-up fee of $239.00 (includes tax)
